  <dc:title xml:lang="srp">Uticaj težine kliničke slike i neželjenih dejstava antiepileptika na stepen depresije, anksioznosti i kognitivnih poremećaja kod pacijenata sa epilepsijom</dc:title>

  <dc:description xml:lang="srp">Standardno lečenje epilepsija podrazumeva primenu antiepileptičnih lekova koji
mogu imati ozbiljna neželjena dejstva. Značajan deo bolesnika i pored politerapije ima neadekvatnu kontrolu napada, što definiše farmakorezistenciju, a tada su i neželjena dejstva lekova izraženija. Mentalni poremećaji su češći kod bolesnika sa epilepsijom, bilo kao posledica samih napada, udružene patologije ili nezavisnog163 prisustva obe bolesti. Generalni cilj ove studije jeste utvrđivanje uticaja težine kliničke slike i antiepileptika na pojavu mentalnih poremećaja kod pacijenata sa
generalizovanom i parcijalnom, farmakorezistentnom i farmakosenzitivnom
epilepsijom. Studija je sprovedena u okviru Klinike za neurologiju i biće uključeni pacijenti koji se ambulatno i hospitalno leče u okviru ove klinike. U grupi od 150 pacijenata sa epilepsijom, uzrasta od 18 do 65 godina biće utvrđeno prisustvo pacijenata sa farmakorezistentnom epilepsijom a na osnovu poštovanja kriterijuma za farmakorezistenciju. Pacijenti će se podeliti u četiri grupe: pacijenti sa generalizovanom farmakorezistentnom epilepsijom, pacijenti sa generalizovanom farmakosenzitivnom epilepsijom, pacijenti sa parcijalnom farmakorezistentnom epilepsijom i pacijenti sa parcijalnom farmakosenzitivnom epilepsijom. Svima će biti urađeno neuropsihološko testiranje i utvrđeno eventualno prisustvo mentalnih
poremećaja primenom skala anksioznosti i depresije, kao i kognitivni status.
Utvrdiće se prisustvo neželjenih efekata antiepiletika i učestalost i vrsta napada na pojavu psihijatrijskih poremećaji. Pacijenti će se pratiti u periodu od 8 meseci, redovnim testiranjem na 8 nedelja: pojava i vrsta napada, učestalost i kontrola napada putem dnevnika napada, kao i evaluirati psihički status skalama, pratiti neurološki nalaz i kognitivni status pacijenata uz korelaciju sa antiepileptikom koji pacijent uzima. Studija će pokazati koji mentalni poremećaji i sa kakvom učestalošću se
javljaju kod pacijenata sa generalizovanom i parcijalnom farmakorezistentnom
epilepsijom u odnosu na bolesnike sa farmakosenzitivnom bolešću kao i da li češći napadi i kombinacije lekova sa naglašenim neželjenim dejstvom na psihičku sferu potenciraju ili izazivaju češće psihijatrijske poremećaje.</dc:description>
  <dc:description xml:lang="eng">Standard treatment of epilepsy involves the use of anti-epileptic drugs that can have serious side effects. A significant part of the patients, in addition to politotherapy, have inadequate control of the attack, which defines pharmacotherapy, and then the undesirable effects of drugs are more pronounced. Mental disorders are more common in patients with epilepsy, either as a result of attacks themselves, associated pathology, or the independent presence of both diseases. The overall goal of this study is to determine the effect of the clinical picture and antiepileptic severity on the occurrence of mental disorders in patients with generalized and partial, pharmacodynamic and pharmacosensitive epilepsy. The study will be conducted within
the Neurology Clinic and will include patients who are ambulately and clinically treated within
this clinic. In a group of 150 patients with epilepsy, aged 18 to 65 years, the presence of patients
with pharmacotherapy epilepsy will be determined based on the compliance with the criteria
for pharmacovigilance. Patients will be divided into four groups: patients with generalized
pharmacotherapeutic epilepsy, patients with generalized pharmacosensitive epilepsy, patients
with partial pharmacotherapy epilepsy, and patients with partial pharmacosensitive epilepsy.
Neuropsychological testing will be performed and all possible mental disorders will be detected
using anxiety and depression scale, as well as cognitive status. The presence of undesirable
effects of anti-epileptics and the frequency and type of attack on the occurrence of psychiatric
disorders will be determined. Patients will be monitored for a period of 8 months by regular 8-
week testing: the onset and type of attack, frequency and control of the attack by the attack log,
as well as evaluate the psychological status of the scales, monitor the neurological findings and
the cognitive status of patients in correlation with the patient&apos;s antiepileptic drug. The study
will show which mental disorders and with what frequency occur in patients with generalized
and partial pharmacoresistence epilepsy in relation to patients with pharmacosensitive disease,
and whether more frequent seizures and combinations of drugs with pronounced adverse
effects in the psychic sphere potentiate or cause more frequent psychiatric disorders.</dc:description>
